ALL UNITS WILL BE ASSESSED A $450.00 DECONTAMINATION FEE IF UNITS ARE RECEIVED AND FOUND TO HAVE NOT BEEN CLEANED THOROUGHLY. THE DECONTAMINATION PROCESS INCLUDES CLEANING BOTH THE INTERIOR AND EXTERIOR OF THE UNIT. IF YOUR CENTRIFUGE IS ASSESSED A DECONTAMINATION FEE AND YOU OPT TO NOT HAVE THE UNIT REPAIRED, THE DECONTAMINATION FEE STILL STANDS.


Flat-Rate repair service repairs all defective parts found. Further inspection and repair of common failure parts are done as deemed necessary. Centrifuge rotor/inserts, and external power supplies are not covered under the flat-rate service. If non-covered items are found to be defective or missing, Ozark Biomedical will notify the customer of their findings and offer replacement parts if available.

In the event a non-covered part is needed to complete a flat-rate repair, Ozark Biomedical's service technician will contact the customer with their findings. The customer can either agree to replace the non-covered, defective part or the centrifuge will be returned to the customer unrepaired. Unrepaired units are subject to any shipping charges.

Any parts that are not covered by the flat-rate service will be charged in addition to the flat-rate service fee.

Ozark Biomedical's customer service will provide an RGA number to you via the email address provided, or by text message if requested, so please ensure all contact information submitted is accurate.

DO NOT SEND ANY ITEMS UNTIL AN RGA NUMBER IS PROVIDED BY CUSTOMER SERVICE!

Upon receiving an Ozark Biomedical RGA number, please ensure the RGA number is clearly visible on the outside of the packaging material. If an RGA number is not clearly visible on the outside of your package, Ozark Biomedical will not accept the package when it arrives at Ozark Biomedical's facility.

IF REQUESTING SERVICE FOR MULTIPLE UNITS, EACH UNIT MUST BE ASSIGNED A SEPARATE RGA NUMBER AND PURCHASE ORDER NUMBER. THE SAME PO NUMBER CANNOT BE USED ON MULTIPLE UNITS.
