IF YOUR CENTRIFUGE IS ASSESSED A DECONTAMINATION FEE AND YOU OPT TO NOT HAVE THE UNIT REPAIRED, THE DECONTAMINATION FEE STILL STANDS. Ozark Biomedical's customer service will provide an RGA number to you via the email address provided, or by text message if requested, so please ensure all contact information submitted
is accurate. DO NOT SEND ANY ITEMS UNTIL AN RGA NUMBER IS PROVIDED BY CUSTOMER SERVICE! Upon receiving an Ozark Biomedical RGA number, please ensure the RGA number is clearly visible on the outside of the packaging material.
If an RGA number is not clearly visible on the outside of your package, Ozark Biomedical will not accept the package when it arrives at Ozark Biomedical's
facility.If you are having difficulties submitting the form online, click here for the printable form to submit via FAX to customer service for RGA approval. In the event you have not received an RGA within an hour of submitting the request, fax the completed form to 501-882-2122.
IF REQUESTING SERVICE FOR MULTIPLE UNITS, EACH UNIT
MUST BE ASSIGNED A SEPARATE RGA NUMBER AND PURCHASE
ORDER NUMBER. THE SAME PO NUMBER CANNOT BE USED ON
MULTIPLE UNITS.
