This is an Evaluation Service option for units that Ozark Biomedical does not offer a flat-rate repair service. In the event the repair amount is greater than $500, or the centrifuge cannot be repaired, an Ozark Biomedical Service Technician will contact you.

To begin your Repair Estimate Process, follow these steps.

1. Check to see if your unit is not supported. Some models do not ship without damaging, there is limited support available, or cannot be shipped due to their large size. Click Here for the short list of unsupported units.

2. If supported add the above service to your cart and proceed to checkout. Filling out the form. You can check out as a guest or if you create an account this form will auto populate on future repairs. After completion submit the order. You will get a thank you for your order notice. Your order has been submitted.

3. Customer service will contact you with an RGA# via email or txt depending upon your selection.

 4. Do NOT ship centrifuge with rotor installed. Include any external power supplies.

 5. Decontaminate your centrifuge. Units sent in without decontamination will be charged an additional $450.00 fee. If the unit is severely contaminated it will be returned unrepaired.

6. Remove rotor and package separately and ship with unit in one box.

7. Put RGA# on outside of box.

8. The $500.00 is applied towards your repair of the unit or towards the evaluation fee if you decided to not repair.
